PHYSICAL EXAMS: NYS requires examinations for all students in grades 1,3,5,7,9 and 11. Exams may be completed by your child’s physician or our school physician, Dr. Emile Benardot, will complete the exam at school.

Flu (Influenza)

Influenza (the flu) is an infection of the nose, throat, and lungs caused by influenza viruses. Many different influenza viruses are constantly changing. Flu viruses cause illness, hospital stays and deaths in the United States each year.

The flu can be very dangerous for children. Each year about 20,000 children younger than 5 years old are hospitalized from flu complications, like pneumonia. MORE INFORMATION CAN BE FOUND HERE

student’s individual needs. Please be sure we have the most up-to-date health information for your child. Please be sure to let us know if there has been a change of phone number or person to call if parents are unavailable.

Medication Authorization form

Did you know that an authorization form must be completed and new form must be completed each school year if your child receives medication (prescription or over the counter) during the hours he/she is attending school? Please remember to pick up meds at the end of the school year.
